Position Summary: We care about the individuals and families living within our community and it is our priority to ensure access to services as soon as possible. The Case Manager provides case management related activities and supports individuals and families throughout treatment. The Case Manager ensures individuals and families are connected with appropriate services using evidence-based clinical practices and interventions, that are consistent with the person’s values, preferences, and individualized needs. As part of our team, this person shares responsibility for directing, coordinating, and managing care and services

Specific Duties/Responsibilities:

  • Demonstrates commitment to the person-centered and family-centered treatment planning process
  • Works with individuals and families to ensure needs are being met within the home, school, and community, and that they have access to the resources they need
  • Responsible for the coordination of services, including medical, psychological, and social care
  • Assists individuals and families with navigating systems of care and accessing appropriate services and level of care
  • Collects demographic and contact information, the person’s use of alcohol and other drugs, and other risk factors, including suicidal or homicidal ideation or behaviors to determine the need for emergency or urgent care
  • Completes level of care assessments and screening tools with individuals and families as appropriate
  • Determines the individualized needs of people seeking FSRI services and connects them with programs internally and externally as appropriate
  • Obtains necessary consents and release of information and participates in care coordination and treatment planning activities
  • Gathers information necessary to identify critical service needs and/or determine when a more intensive service is necessary
  • Assesses emergency situations and facilitates immediate access to stabilization and harm reduction activities
  • Ensures individuals and families are offered appointment times during times that meet their needs
  • Coordinates with our Emergency Services/Mobile Crisis staff and/or DCO for the person to receive an Emergency Evaluation, within 1 hour, to ensure the prioritization of this person’s safety as appropriate
  • Assists with warm hand offs to our Emergency Services/Mobile Crisis staff and DCO in these situations to promote seamless transitions of care for the people receiving our services
  • Provides individuals and families with psychoeducation regarding our Emergency & Crisis Services meet 988 Suicide & Crisis Lifeline
  • Completes all session notes within 48 hours from the date of the contact or service
  • Completes all required documentation within designated timeframes, including data collection and entry as required
  • Assists with data entry as appropriate
  • Maintains contact with individuals, families, and referral sources to ensure requested assistance and referrals are completed      
  • Participates in outreach activities as appropriate
  • Provides language support and translation services in collaboration with internal programs and community partners as appropriate
  • Completes follow up calls and coordinates care as appropriate
  • Participates in ongoing supervision and staff meetings 
  • Assists with administrative coordinator duties as appropriate, including covering front office tasks to support open access model
  • Works collaboratively across department and divisions to ensure quality service
  • Participates in grant activities such as CAB as appropriate
  • Contributes to a healthy and inclusive staff culture through modeling inclusion, flexibility, compassion, and a strong work ethic
  • Provides transportation for clients as appropriate
  • Adheres to all agency and CCBHC policies and procedures, including HIPAA, 42 CFR Part 2, and all state and federal regulations  
  • Performs additional responsibilities consistent with Agency needs, defined in consultation with supervisor(s)

Qualifications:

  • Bachelor's degree level education or equivalent experience
  • Behavioral Health experience preferred
  • Skilled in operating various medical record software and hardware, word-processing, and database software programs
  • Spanish Speaking preferred and is compensated
  • Excellent multitasking and communication skills
  • Ability to work independently and part of a team
  • Bilingual/ASL skills are compensated by an additional 6%, above base pay.
  • Multilingual skills are compensated by an additional 8%, above base pay.

Physical Requirements: This position requires residential and community visits, employees in this position must have the ability to:

  • Current driver’s license, reliable transportation, registration and auto insurance   
  • Ability to communicate effectively    
  • Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators  
  • Ability to lift up to 20lbs 
  • Must be able to work remotely and in person adhering to PHI requirements
