FSRI- Community Health Worker, DCYF Caring Dads

Position Summary: The CHW will work as part of a multidisciplinary team to support referrals to the Caring Dads program. CHW is responsible for engaging Fathers; completing intake documentation; Social Determinants of Health screenings and other screening tools; identifying areas of need; ensuring referrals are made to the appropriately matched services. CHW will co-facilitate a specialized 17-week group intervention meeting for referred Fathers aimed to promote healthy family interaction, active listening, child-centered parenting, and foster non-abusive, co-parenting relationships. Additionally, CHW will serve as "Mother Contact" connecting with the identified partner to discuss Caring Dads program components, complete Social Determinants of Health screening as needed, and support care coordination to the appropriately matched services and provide ongoing support and monitoring as needed.
Qualifications:
- Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
- Community Health Worker Certification preferred
- Ability to meet all requirements to be certified as a Community Health Worker within the first 12 months of employment
- Bilingual in Spanish or Portuguese preferred
- Strong interpersonal and social skills with an ability to collaborate with a variety of community based organizations, coalitions and stakeholders
- Prior experience in group facilitation, victim advocacy, domestic violence offenders and/or crisis intervention preferred
- Commitment and sensitivity to issues related to the needs of parents and children, as survivors of Domestic Violence
- Ability to work independently and as an interdependent team member
- Ability to provide services at agency sites, community locations, and virtually as required
- Ability to have a flexible schedule to include some evenings
- Must pass RI BCI, National Fingerprint & DCYF CANTS check
- Valid driver’s license and reliable transportation
